Job summary

OneWell Health Care is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing services in home and community settings for individuals with Intellectual and Developmental Disabilities (IDD) in Pennsylvania. The role operates under the supervision of an RN Case Manager and focuses on implementing care plans, administering medications, and promoting health, safety, and quality of life.

Qualifications

• Current LPN license in Pennsylvania.
• CPR and First Aid certification required.
• Minimum of 1 year nursing experience.
• Experience supporting individuals with IDD, home health, or long-term care preferred.

Responsibilities

• Administer prescribed medications and treatments.
• Implement individualized care plans under RN supervision.
• Perform clinical procedures including wound care, catheter care, feeding tube management, vital signs.
• Provide education and training to families, caregivers, and new staff.
• Maintain accurate documentation and participate in required trainings.
