OneWell Health Care in New Providence, PA is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services to individuals with IDD in home and community-based settings across Pennsylvania. You will work under RN Case Manager supervision, implementing individualized care plans, administering medications, and supporting families to maintain health and safety.

Qualifikationen

Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
Strong nursing skills for IDD home health settings.
Ability to work independently under RN supervision.

Perform clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring.
