Job summary

OneWell Health Care invites a dedicated Licensed Practical Nurse (LPN) in York, PA to deliver skilled nursing in home and community settings under RN supervision. You will implement care plans, administer medications, and support participants and families to protect health, safety, dignity, and independence.

This field-based role requires PA licensure, CPR and First Aid, and a minimum of 1 year of nursing experience; IDD/home health experience is preferred.

Qualifications

• Active LPN license in Pennsylvania.
• CPR and First Aid certification required.
• Minimum 1 year nursing experience.
• Experience with IDD/home health preferred.

Responsibilities

• Implement individualized care plans; administer prescribed medications and treatments.
• Perform clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring.
• Educate families, caregivers, and new staff; participate in interdisciplinary meetings and case reviews.
