Qualifications

• Master's Degree in Social Work required.
• MSW licensure/eligibility per Michigan at hire.
• 2 years of health care related experience preferred.
• Care management experience preferred.
• Experience in assessment, crisis intervention, grief and discharge planning preferred.

Responsibilities

• Collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs.
• Coordinate alternate sites of care and discharge planning; address abuse/neglect, adoption, substance use disorders and end of life decisions.
• Conduct psychosocial assessments and execute care plans for identified needs.
• Communicate with alternate care sites and community agencies to address patient needs.
• Document assessments and plans in EMR reflecting current condition and imperatives.
• Provide consultation and resources to healthcare team; maintain knowledge of community resources.
• Participate in department or regional committees; stay current on federal/state regulations.
• Provide short term therapeutic support as appropriate for setting and location.
