Job summary

Corewell Health is seeking a Social Worker to collaborate with an interdisciplinary team to support patients and families with complex psychosocial needs, including discharge planning, guardianships, and bereavement resources.

Responsibilities include conducting assessments, arranging referrals, documenting plans in the EMR, and providing resources to the care team while staying current with relevant regulations and community resources.

Qualifications

• Master's degree in Social Work required.
• 2 years of healthcare-related social work experience preferred.
• Care management experience preferred.
• Experience in individual/family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• MSW license in Michigan upon hire required.
• Driver's license – Michigan required.

Responsibilities

• Collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs.
• Coordinate alternate site care and discharge planning; address abuse/neglect; facilitate referrals and resources.
• Document assessments and plans in the EMR reflecting current condition and psychosocial imperatives.
• Provide consultation and resources to healthcare team members.
• Maintain current knowledge of community resources and participate in stakeholder committees as assigned.
• Stay updated on federal/state regulations relevant to the role and provide short-term therapeutic support when appropriate.
