• Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred
• MSW - STATE_MI licensure upon hire

Responsibilities

• Collaborate with interdisciplinary team to assist patients and families with complex psychosocial needs.
• Coordinate alternate sites, discharge planning, and resource referrals.
• Conduct psychosocial assessments and execute care plans post discharge.
• Communicate with alternate care sites and community agencies.
• Document assessments and plans in the EMR reflecting current conditions.
• Provide consultation and resources to healthcare team.
• Maintain knowledge of community resources and participate in committees as requested.
