Job summary

Corewell Health is seeking a part-time Social Worker to collaborate with an interdisciplinary team in addressing complex psychosocial needs and discharge planning for patients and families. Responsibilities include coordinating with post-discharge sites, assessing patients, and providing resources to the care team.

The role covers inpatient psychiatric duties as applicable, with focus on securing appropriate referrals and maintaining up-to-date knowledge of community resources and regulations.

Qualifications

• Master’s degree in Social Work required.
• Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.

Responsibilities

• Assist patients and families with complex psychosocial needs and discharge planning.
• Coordinate with alternate sites of care and community agencies.
• Document assessments and plans in the EMR.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources and regulations.
• Support inpatient psychiatric duties as applicable.
