Job summary

Signify Health, part of CVS Health, seeks a Part-Time Physician to provide in-Home Health Evaluations and engage with patients in their homes. The role involves a 45-minute visit, medical history review, physical evaluation, and optional diagnostic tests as ordered by the health plan.

Clinicians will travel locally, may conduct virtual visits, and are eligible for comprehensive benefits. Licensure in coverage areas is required; multi-state licenses are a plus.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
