Job summary

Qualifications

• Active, unrestricted medical license in coverage area.
• Multi-state licenses are preferred.
• Part-time availability and ability to travel locally as needed.

Responsibilities

• Visit patients in their homes for In-Home Health Evaluations, including health questions, medication history, and physical evaluation.
• Use iPad and clinical workflow to document evaluations and coordinate care.
• Provide Signify Health services such as Diagnostic & Preventive Services as indicated.
• Engage with health plan members and connect them to the right care.
• Travel locally to licensed states as required and conduct virtual visits when needed.
