Job summary

Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide in-home health evaluations, engaging with members in their homes to bridge gaps in care.

As an in-home clinician, you will conduct evaluations lasting about 45 minutes, review medications and history, and perform physical assessments. Travel locally within licensed states and occasional virtual visits are expected.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations and engage with patients in their homes.
• Review medications and medical history, perform physical evaluations, and conduct needed tests as per health plan orders.
• Travel locally and within licensed states; conduct virtual visits as needed.
