Job summary

Signify Health, part of CVS Health, seeks a part-time physician to perform in-home health evaluations at patients' residences across licensed areas. The clinician will review med histories, conduct physicals, and coordinate with the patient's health plan and PCPs as needed.

Visits are around 45 minutes, with optional virtual visits, and the role requires local travel within licensed states. Excellent communication and compassionate patient care are essential.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses are preferred.

Responsibilities

• Conduct In-Home Health Evaluations for patients.
• Review medication and medical history, perform physical evaluation.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Coordinate with health plans and care teams to bridge gaps in care.
• Provide diagnostic and preventive services as indicated.
