Job summary

Signify Health, part of CVS Health, is seeking a Part-Time Physician to deliver in-home health evaluations, meeting patients in the comfort of their own homes. You will help bridge gaps in care and build trusted relationships to improve health outcomes.

The visits last about 45 minutes and include health questions, medication and history review, and a physical exam. You may travel locally and across licensed states, and virtual visits are supported.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations and engage with patients in their homes.
• Perform medication and medical history reviews and physical evaluations.
• Utilize iPad and clinical workflow; no prescription changes unless directed by health plan.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Build trusted relationships to improve health outcomes and connect patients to appropriate care.
