S Soleo Health is seeking a Part-time Infusion Nurse to support infusion patients in the Greeley, CO area. The role includes direct patient care, home infusion therapy, and education, with a flexible 30–39 hour schedule weekly. Travel up to 2 hours is required, with mileage reimbursement and hourly pay.

Qualifications

• Active RN license in good standing with Colorado Board of Nursing.

Responsibilities

• Provide direct patient care in home/in alternate sites, including infusion therapy.
• Educate patient and care partners on infusion procedures and safety.
• Assess home environment and implement safety interventions.
• Develop and evaluate care plans with the team.
• Coordinate patient care using approved platforms and document accordingly.
• Participate in on-call rotation for 24/7 nursing support.
