Qualifications

• Licensed PTA in the state where assigned.
• Graduate from a CAPTE-accredited PTA program.
• One year related PTA experience for home health or approval by regional leaders if less than one year.
• Current CPR certification.

Responsibilities

• Provides physical therapy services and interventions under supervision of a PT.
• Observes and reports patient responses to treatment to the supervising PT.
• Documents supervisory activities in the medical record.
• Monitors patient progress and documents changes in condition.
• Performs duties as assigned and coordinates care with the agency.
