Aveanna Healthcare is seeking a Physical Therapist (PT) Part Time PRN to join our home health care team serving Birmingham, AL. You will assess mobility, develop and monitor a plan of care, and deliver therapy in patients' homes with minimal supervision.

Requirements include a PT degree, active license in the state, and CPR certification. Preferred candidates have home health experience and strong organizational skills to support patient outcomes and caregiver education.

Qualifikationen

• Degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in the state of practice
• Valid CPR

Aufgaben

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
• Deliver physical therapy services per physician orders and assess progress toward goals.
• Maintain and update plan of care with goals, progress and expected outcomes.
• Assist with discharge planning and home exercise program education for patients and caregivers.
• Coordinate with healthcare team and supervise PTA/HHA as applicable.
