Job summary

ProHealth Home Health is seeking a Registered Physical Therapist (PT) to provide intermittent home-based therapy in Calhoun County, Alabama. The PT will follow physician orders, assess patients, and contribute to individualized care plans.

Qualified candidates hold an Alabama PT license and have strong clinical evaluation skills, with 2 years of experience preferred. This is a part-time, on-site role based in Anniston, AL.

Qualifications

• Graduate from a PT curriculum approved by accrediting bodies.
• Currently licensed to practice in Alabama.
• Two years’ experience preferred.

Responsibilities

• Administers physician-prescribed physical therapy in patients' homes.
• Performs skilled procedures and initial evaluations.
• Documents patient progress and writes reports to physicians.
• Maintains patient care plans and coordinates with the care team.
• Assists in developing the plan of care and contributes to quality improvement.

Education

Graduate from PT curriculum approved by ABPTS/ACOTE/CAAHEA or ACMEC/AMSA office

Alabama PT license

Two years’ experience (preferred)
