Job summary

Aveanna Healthcare is seeking a Physical Therapist (PT) to join our home health team in Alabama. The role involves evaluating mobility, creating plans of care, and delivering PT services in patients' homes.

The position requires a PT degree, an unrestricted license, CPR, and reliable transportation; OASIS documentation is preferred. Territory includes Jackson County and Lacey Springs.

Qualifications

• Degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in the state of practice
• Valid CPR, Driver's License and Proof of Automobile Insurance

Responsibilities

• Perform initial and ongoing mobility assessments to evaluate function.
• Provide written evaluation and plan of care defining PT needs and outcomes.
• Delivery of physical therapy services per physician orders.
• Discharge planning and home exercise plan education for patients and caregivers.
• Complete OASIS assessments where allowed by state regulation.
