ProHealth Home Health in Anniston, AL is seeking a Registered Physical Therapist (PRN) to provide mobility-focused therapy in patients' homes under physician orders. You will perform initial evaluations, monitor progress, and educate patients and families on home exercise programs.

The role requires current AL PT licensure, the ability to work independently, and travel to patient homes within Calhoun County. This on-site, part-time position aligns with agency goals and patient-centered care.

Formación

• Graduate from a physical therapist program accredited by CAPTE or equivalent.
• Currently licensed as a Physical Therapist in the practicing state.
• Two years’ experience preferred; ability to work independently.

Responsabilidades

• Provides physician-prescribed physical therapy to patients in their homes.
• Performs initial evaluations and documents patient progress.
• Writes reports to physicians regarding patient progress and care plans.
• Instructs patients and family in home exercise programs.
• Participates in QA/PI activities and in-service programs.
• Completes and submits OASIS assessments and related documentation.
• Directs and supervises home health personnel as required.
• Assists physicians with treatment plan adjustments as needed.
