Job summary

Aveanna Healthcare in Alabama is seeking a Part Time PRN Licensed Physical Therapy Assistant to perform physical therapy treatments under the supervision of a Physical Therapist.

You will administer care in patients' homes within Jefferson and Shelby Counties, document progress, assess home safety, and participate in interdisciplinary team meetings while maintaining privacy standards.

Qualifications

• Current license as a Physical Therapy Assistant in GA.
• Graduate of an accredited PTA program.
• Preferably 1 year of home health care clinical experience.

Responsibilities

• Administers physical therapy to patients following the Plan of Care developed by the Physician and Physical Therapist.
• Evaluates and documents effectiveness of physical therapy services to the patient and his/her response to treatment.
• Assesss the home environment to identify safety hazards and recommends changes to reduce risks.
• Recognizes and identifies need for initiation of services from other disciplines; communicates with supervisor and refers as needed.
• Assesses social, emotional and physical factors within the patient’s home affecting care and reports to supervising Physical Therapist.
• Attends interdisciplinary team meetings and helps modify the Plan of Care as needed.
• Reports adverse changes in patient condition to the supervising Physical Therapist or Care Manager.
• Completes daily assignments within a reasonable time frame.
• Attends inservices to keep skills up to date and maintains privacy and security practices.
