Job summary

Aveanna Healthcare is seeking a licensed Physical Therapist to join our home health team in Alabama. You will assess and treat patients in their own homes, develop plans of care, and coordinate with families and the health care team.

This part-time role covers Etowah, Calhoun, Cherokee, Calhoun and Marshall counties with Monday–Friday visits. OASIS documentation experience is preferred; CPR and valid driver’s license required.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR and driver's license.
• Experience in Home Health Therapy and OASIS documentation is preferred.

Responsibilities

• Perform initial and ongoing mobility assessments to evaluate mobility, coordination, strength and function.
• Provide a written evaluation and plan of care defining physical therapy needs, goals and outcomes.
• Deliver PT services to comply with physician orders.
• Complete OASIS assessments where allowed by state practice regulations and coordinate discharge planning as appropriate.
• Maintain accurate documentation and communicate with patients, families and healthcare team.
