Job summary

Aveanna Healthcare in Birmingham, AL seeks an Occupational Therapist (OT) Part Time PRN to assess, teach, and deliver therapy in the home health setting under a physician-approved plan of care. You will collaborate with an interdisciplinary team to optimize patient well-being and discharge readiness.

Responsibilities include initial and ongoing mobility assessments, planning and documenting care, delivering therapy per orders, and coordinating with families and care staff.

Qualifications

• Degree in Occupational Therapy from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapist in the state of practice.
• Valid CPR.

Responsibilities

• Perform initial and ongoing mobility, coordination, strength assessments.
• Delivery of occupational therapy services to comply with prescribed physician orders.
• Document progress and plan of care, and determine discharge plans as appropriate.
• Maintain timely documentation and modify goals as needed.
• Discharge planning and education of patient and caregivers on home exercise plan.
• Coordinate services with health care team, physicians, parents/guardians, and nurses.
• Supervise other personnel (OTA, HHA) as applicable.
• Complete OASIS assessments where state requirements allow.
