Aveanna Healthcare in Birmingham, AL is seeking a Registered Nurse (Home Health) for part-time PRN work. You will provide one-on-one in-home nursing care, perform assessments, develop care plans, and coordinate with physicians to promote recovery, safety, and independence for patients in their homes.

This role emphasizes patient-centered care, education for patients and caregivers, timely documentation, and collaboration with a multidisciplinary team.

Qualifications

• Active RN license in the state of practice.
• Patient-focused, organized, and motivated by delivering high-quality care.
• Current driver’s license and reliable transportation.
• CPR certification (or ability to obtain).

Responsibilities

• Serve as the primary clinical partner for patients and families, delivering individualized, one-on-one nursing care in the home that promotes recovery, safety, and independence.
• Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, and translate those findings into thoughtful, patient-centered care plans.
• Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, postoperative and chronic disease management as appropriate.
• Educate and empower patients and caregivers through clear instruction, coaching, and support for managing symptoms and daily care at home.
• Monitor patient progress, identify changes in condition, and communicate with physicians and the care team to adjust plans and prevent hospitalizations.
• Coordinate care across providers to ensure continuity and high-quality outcomes across the home health journey.
• Complete timely and accurate documentation reflecting the clinical picture and supporting quality and compliance.
