• RN licensure in designated state(s) as appropriate.

Responsibilities

• Coordinate care from admission to discharge with goal-directed plans.
• Perform on-site supervisory visits and complete follow-up documentation.
• Take on-call duties as assigned for timely patient care.
• Uphold CHAP standards and ensure compliant nursing services.
• Review medical records for accuracy and regulatory adherence.
• Analyze quality trends and support improvement plans.
• Provide education and collaboration with interdisciplinary teams.
