Aveanna Healthcare is seeking a Nursing Supervisor (RN) to lead clinical staff in Minnesota. The role combines internal case management, start-of-care assessments, and field visits to homes, with a strong emphasis on mentoring and ensuring high-quality care.

The position supports branch office operations, hybrid work arrangements, and collaboration with operations to maintain regulatory compliance and drive patient outcomes in the community.

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year hands-on nursing experience.
• Willing to travel to patients’ homes in designated territory.
• Reliable transportation and valid driver’s license; pass MVR.
• Current CPR certification.

Responsibilities

• Internal case management, evaluation, education and development of nursing talent.
• Transition of patients from facility to home through Start of Care Assessments and Re-evaluations.
• Review physicians’ orders and update plans of care and documentation as needed.
• Assist with selecting, training and validation of new clinical staff.
• Promote and enhance clinical expertise to provide safe, quality nursing care.
• Collaborate with operations and business development teams.
• Ensure adherence to State, Federal, Local regulations and Aveanna policies and procedures.
