Job summary

The Osborne Association is seeking a part-time Elder Reentry Initiative (ERI) Mentor to support adults aged 50+ transitioning from incarceration into the community. You will apply CTI with a structured, time-limited case management approach, focusing on housing, benefits, and health needs.

The role requires building rapport quickly and coordinating warm handoffs to long-term providers, ensuring reentry stability and independence.

Qualifications

• Associate’s Degree in Communications, Human Services or a related field.
• Or a high school diploma with 1-2 years of mentorship, counseling, case management, or transitional support experience.

Responsibilities

• CTI-Based Mentorship: manage a caseload of ~16 older adult participants.
• Assessment & Phase Planning: complete evaluations and target 1-3 stability areas.
• Community Navigation & Warm Handoffs: connect participants to community partners.
• Behavioral & Emotional Support: provide trauma-informed guidance and support.
• Crisis Support & Availability: offer after-hours/crisis support as needed.
• Flexible Appointment Assistance: schedule may include early mornings or evenings.
• Interdisciplinary Collaboration: participate in CTI team supervision.
• Partnership Development: build relationships with community agencies.
• Data & Documentation: maintain timely notes and closing reports.
• Training & Collaboration: attend professional development trainings.
