Job summary

Osborne Association in New York is seeking a full-time Peer Mentor for the Elder Reentry Initiative (ERI) to support adults 50+ transitioning from incarceration. You will guide participants through structured CTI phases, helping them secure housing, benefits, and health or behavioral supports, while building rapport and coordinating with partners.

Under supervision, you will document progress, maintain caseloads, and participate in weekly CTI meetings.

Qualifications

• Associate’s Degree in Communications, Human Services or related field or equivalent experience.
• Experience in mentorship, counseling, case management, or transitional support services preferred.
• Bilingual English/Spanish proficiency preferred.

Responsibilities

• CTI-based mentorship for ~20 older adults through Transition, Try-Out, and Transfer phases.
• Assess and plan Phase goals targeting housing, benefits, or treatment.
• Connect participants to community partners and accompany to initial appointments.
• Provide trauma-informed guidance and motivational interviewing throughout reentry.
• Offer on-call crisis support and flexible weekday scheduling as needed.
• Maintain documentation in electronic systems and participate in CTI supervision.
