Job summary

Osborne Association seeks a Care Manager in NYC to coordinate the CTI-based reentry plan for older adults (50+), collaborating with a Clinical Social Worker and two Peer Mentors. You will oversee assessments, planning, and crisis support to ensure safe transitions from incarceration back to the community.

The role requires trauma-informed practice, strong coordination skills, and the ability to engage diverse populations while working flexible hours and traveling locally as needed.

Qualifications

• Associate’s Degree or equivalent with 1–2 years in mentorship, case management, or transitional support services.
• Experience with CTI or trauma-informed care is preferred.
• Bilingual English/Spanish is highly preferred.

Responsibilities

• Act as central coordinator for the CTI team, guiding steps across Transition, Try-Out, and Transfer phases.
• Perform detailed participant screenings and create individualized transitional plans.
• Lead interdisciplinary coordination and collaborate with the Clinical Social Worker and Peer Mentors.
• Provide trauma-responsive, strengths-based case management for older adults preparing to reenter the community.
• Cultivate partnerships with community service providers in aging, housing, health care, and reentry.
• Participate in on-call crisis support and occasional early morning or late-evening appointments.
• Attend training and collaborate in-person as required by the role.
