Job summary

Osborne Association in New York seeks a full-time Peer Mentor for the Elder Reentry Initiative (ERI). Grounded in the CTI model, the mentor will support adults age 50+ as they transition from incarceration to community life, focusing on housing, benefits, and connection to local services.

You will manage a caseload of about 20 participants, document progress in our systems, participate in weekly CTI supervision, and collaborate with clinicians, care managers, and external partners.

Qualifications

  • Associate’s Degree in Communications, Human Services or a related field; or high school diploma with 1–2 years of mentorship, counseling, case management, or transitional support experience.

Responsibilities

  • CTI-Based Mentorship: manage a caseload of approximately 20 older adult participants through CTI phases to support reentry.
  • Assessment & Phase Planning: collaborate with clinical team to complete evaluations and plan Phase plans targeting housing, benefits, or treatment.
  • Community Navigation & Warm Handoffs: connect participants to community partners and accompany them to initial appointments.
  • Behavioral & Emotional Support: use motivational interviewing and trauma-informed approaches to support participants.
  • Crisis Support & Availability: provide on-call crisis support, including after-hours and weekends.
  • Flexible Appointment Assistance: assist participants during appointments that may start early or late.
  • Interdisciplinary Collaboration: participate in CTI supervision meetings and coordinate with Care Manager and Clinician teams.
  • Partnership Development: build relationships with community-based agencies to expand resources.

Education

Associate’s Degree in Communications, Human Services or related field
HS Diploma or equivalent with 1-2 years of related experience
