Job summary

Heartshare is seeking a Direct Support Professional to implement person-centered services for individuals with developmental disabilities in New York. The role focuses on implementing service plans, promoting community inclusion, and ensuring the physical and emotional well-being of participants.

Responsibilities include supervising activities, documenting progress, and collaborating with families and providers.

Qualifications

• High School Diploma or GED; CNA strongly preferred but not required.
• One year with individuals with developmental disabilities preferred.
• NYS driver’s license preferred but not required.

Responsibilities

• Implement individuals’ plans aligning with services, goals and outcomes.
• Engage individuals in skills development, socialization and task participation.
• Promote community inclusion and form friendships.
• Maintain communication with family, advocates, and providers.
• Transport individuals or assist with public transportation as needed.

Education
