Heartshare is seeking a Direct Support Professional (DSP) in New York to implement person-centered services for individuals with developmental disabilities. The DSP will ensure plans, goals and outcomes are carried out in both program and community activities, and support the individual’s overall well-being.

The role involves engagement in skills development, socialization, and task participation, promoting community inclusion, assisting with transportation, and documenting data.

Qualifications

• AMAP, CPR/First Aid and SCIP certifications are required or pursued during employment.
• High School Diploma or GED preferred but not required; CNA preferred.

Responsabilités

• Implement individuals’ plans consistent with services, goals and outcomes.
• Plan and implement program and community activities addressing goals.
• Support and teach individuals according to their services, goals and outcomes.
• Engage individuals in skill development, socialization and task participation.
• Promote inclusion in the community according to individual needs and interests.
• Model positive interactions and foster friendships within the community.
• Assist with transportation or use of public transit as needed.
• Participate in PPT meetings and document information accurately.
• Ensure timely completion of written reports and maintain confidentiality.
• Provide appropriate supervision and train staff on LOS and program expectations.
• Assist with activities such as toileting, showering, meal prep, and daily living tasks as needed.
• Report incidents promptly to program management and maintain a safe environment.
