Job summary

Heartshare Wellness LTD is seeking a Direct Support Professional to implement person-centered services and support the physical and emotional well-being of individuals with developmental disabilities. You will help plan and deliver activities in both program and community settings, fostering growth, independence, and social inclusion.

Qualifications

• High School Diploma or equivalent required; CNA preferred but not mandatory.
• One year or more experience with individuals with developmental disabilities preferred.
• NYS driver’s license preferred but not required.

Responsibilities

• Implement individuals’ plans and ensure services, goals and outcomes are delivered.
• Participate in planning and implementing activities in program and community settings.
• Support and teach individuals, promoting skills development and social participation.
• Engage individuals in community interactions and facilitate friendships.
• Provide supervision and ensure safety, reporting incidents promptly.
• Complete all written reports and documentation timely.
