Job summary

Heartshare is seeking a Direct Support Professional in New York to implement person-centered services and support individuals’ well-being. You will deliver services aligned with each person’s goals, and assist with community inclusion and daily activities.

The role requires maintaining certifications (AMAP, CPR/First Aid, SCIP) and may include bi-annual discretionary bonuses in addition to the hourly base rate.

Qualifications

• High School Diploma, GED or CNA strongly preferred but not required.
• One year or more experience working with individuals with developmental disabilities preferred.

Responsibilities

• Implement individuals’ plans and participate in program and community activities to meet goals.
• Support individuals in skill development, socialization, and task participation across settings.
• Promote community inclusion and foster friendships.
• Maintain timely documentation of medical, behavioral, and service goals.

Education

High School Diploma, GED or CNA (Certified Nursing Assistant)

AMAP certification
