Heartshare is seeking a Direct Support Professional in New York, NY to implement person-centered plans and support individuals in daily living, community activities, and skill development. The role includes transporting individuals, documenting progress, and ensuring safety and privacy in all tasks.

Starting hourly base rate is $19 with potential earnings growth through certifications, training, and performance; a comprehensive benefits package accompanies full-time employment in a team-focused

Qualifications

High School Diploma or GED preferred; CNA encouraged.
Experience with individuals with developmental disabilities preferred.
Valid NYS driver’s license preferred.

Responsibilities

Implement individuals’ plans and goals in program and community activities.
Transport individuals or assist with public transportation.
Document information and data related to medical, behavioral, and services goals.
Maintain safety, cleanliness, and confidentiality; report incidents immediately.
Support individuals in skill development, socialization, and community inclusion.
