Job summary

Heartshare in New York seeks a Direct Support Professional (DSP) to implement person-centered services for individuals with developmental disabilities. The DSP will ensure plans and goals are carried out in program and community settings, safeguarding wellbeing and promoting autonomy.

Qualifications

Direct Support Professional duties and responsibilities for individuals with developmental disabilities.
One year or more experience in related settings preferred.
NYS driver’s license preferred but not required.

Responsibilities

Implement individuals’ plans and participate in activities addressing goals and outcomes.
Support and teach individuals in skill development and socialization across settings.
Promote inclusion in community and foster friendships with neighbors and peers.
Maintain communication with family members, advocates, and service providers.
Transport individuals or assist with public transportation as needed.
Participate in planning recreational activities and PPT meetings.
Document data in medical, behavioral and services records with accuracy.
