Job summary

Prairie Community Services is seeking a Direct Support Provider for a part-time on-site role at Eastwood Home in Redwood Falls, MN. You will supervise and care for individuals served, supporting daily living activities, recreation, meals, and community orientation, with a starting wage of $16.75–$17.75 per hour.

Responsibilities include medication management assistance, documentation of daily progress, and following person-centered plans.

Qualifications

• Minimum age 17 or per licensure.
• Pass DHS background clearance.
• Valid driver’s license.
• Read, write, and understand English.
• Read, comprehend, and follow each person served’s program plans.

Responsibilities

• Provide supervision, care and safety to individuals being served.
• Support daily living, recreation, community orientation, medication management, meal preparation, laundry and cleaning.
• Chart daily program information and follow person-centered plans.
