Prairie Community Services in Thief River Falls, MN is seeking a Direct Support Provider (Part-time) to supervise, care for, and support individuals in daily living activities, recreation, and community integration. Starting wage $17.50–$19.50 per hour.

You will follow person-centered plans, manage basic needs, assist with medications per policy, and document daily progress while ensuring safety and dignity for the people served.

Qualifications

• Age 17 or older as required by licensure.
• Must pass DHS background clearance.
• Valid driver's license required.
• Ability to read, write, communicate and understand English.
• Follow person served’s program plans and procedures.

Responsibilities

• Provide supervision, care and safety to individuals served in daily living, recreation and community activities.
• Assist with medication management, meal prep, laundry and cleaning as needed.
• Document daily program information and progress.
• Uphold person-centered approaches and safety standards.
