Job summary

Prairie Community Services in Fairfax, MN is seeking a part-time Direct Support Provider to supervise and assist individuals in daily living, recreation and community activities. The role includes medication management, meal prep, laundry, and safety oversight, with a starting wage of $16.75-$17.75 per hour.

Qualifications

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Support daily living activities, recreation, community orientation, meals, laundry, and cleaning.
• Assist with medication management and document daily program information.
