Job summary

Prairie Community Services is seeking a Direct Support Provider at Edgewood Home in Thief River Falls, MN. This part-time, on-site role involves supervising and supporting individuals in daily living, recreation, and community activities, including medication management and household tasks. Starting wage is $17.50–$18.50 per hour.

The position requires 17+ years of age, background clearance, a valid driver’s license, and English proficiency. Training in person-centered care is provided.

Qualifications

• Must be at least 17 years old or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Assist with daily living activities, recreation and community orientation.
• Support with medication management, meal preparation, laundry and cleaning.
• Maintain accurate charts and daily notes for each person served.
