Job summary

Reserve Care in Cleveland, OH is seeking a Registered Nurse (RN) for the 3rd shift at our New Lakeside Facility. The RN will provide prescribed medical treatment and personal care to terminally ill patients, in collaboration with a transdisciplinary team, and will coordinate the Plan of Care using the Nursing Process.

Key duties include assessing needs, administering intermittent skilled nursing, monitoring pain and symptoms, and participating in care conferences and discharge planning.

Qualifications

• Licensed RN in Ohio.
• Current CPR certification.
• Experience in hospice care or home health is preferred.

Responsibilities

• Provide intermittent skilled nursing services under physician guidance.
• Assess physical, psychosocial, and spiritual needs of patients and families.
• Monitor pain and symptoms; inform and support patients and families.
• Coordinate the Plan of Care and participate in care conferences and discharge planning.
• Maintain plans and reports related to hospice care; participate in on-call rotation.
