Job summary

Monarch Healthcare in North Carolina is seeking a Community Specialist to build positive relationships with individuals receiving services and their families across work, home, school, and community settings.

You will support personal goals, provide respite, and document progress while coordinating with caregivers and staff to ensure the person-centered plan is followed and outcomes are achieved.

Qualifications

• High School Diploma (Required).
• Driver's license (Valid) - USA.
• Experience working with adults with a mental health diagnosis and/or developmental disabilities is not required.

Responsibilities

• Assess and support individuals to identify hopes, dreams and goals.
• Protect rights and promote advocacy, including providing relief to caregivers.
• Serve as a resource on community services and supports for identified needs.
• Supervise and educate individuals in work, home, school, or community settings.
• Provide one-on-one support for emotional, physical, and medical needs.
• Maintain a safe environment by following safety procedures.
• Document progress, goals, outcomes, and incident reports accurately.
• Attend meetings and trainings; maintain required certifications.
• Travel may be required; substitute in-house or in the community as needed.
