Job summary

Monarch Healthcare in North Carolina seeks a Community Specialist to build positive relationships with individuals receiving services and their families in work, home, school, or community settings. You will support personal goals, monitor safety, and ensure rights are respected while coordinating with a care team.

The role includes documentation, staff mentoring, and occasional travel. A High School Diploma is required, and a valid driver’s license is preferred.

Qualifications

• High School Diploma is required.
• Experience working with adults with a mental health diagnosis and/or developmental disabilities is typical for this role.

Responsibilities

• Assess individuals’ hopes and goals and provide supports to help achieve them.
• Protect and promote the rights of individuals receiving services and facilitate advocacy.
• Provide one-on-one support to meet emotional, physical, and medical needs.
• Document progress with daily notes, incident reports, and timesheets.
• Maintain safety and a compliant environment; complete required documentation.
