Primary Health Solutions is seeking a part-time Case Manager in Moraine, OH. You will deliver CPST services to adults 18+, develop collaborative treatment plans, and coordinate with a multidisciplinary team to ensure client-centered care.

The role requires strong time management, ability to work independently in community settings, and experience with mental health services. Reporting to Assistant Managers of Traditional Case Management, this position supports a flexible, compassionate approach.

Formación

• Experience providing case management to adults with severe mental illness, including dual diagnosis.
• Bachelor's or Associate degree as specified with required field experience.
• Ability to read safety rules, maintain procedures, and write routine reports.

Responsabilidades

• Coordinate ongoing assessment and monitoring of clients and families.
• Provide crisis intervention services for clients in acute distress.
• Educate clients about community resources and ensure access to services.
• Monitor service delivery and advocate for coordinated care across providers.
• Deliver education on mental health, symptoms, and medications.
• Promote recovery and natural supports, enhancing daily functioning.
• Lead or co-facilitate therapeutic groups aligned with ISP goals.
• Meet productivity standards and maintain clinical records with >=90% accuracy.
• Complete incident reports and demonstrate proficiency in agency systems.
• Attend required meetings and case conferences.
