Qualifications

• Experience providing case management to adults with mental illness and dual diagnosed populations.
• Ability to develop collaborative, goal-oriented treatment plans and monitor outcomes.
• Knowledge of HIPAA and professional, ethical standards.

Responsibilities

• Coordinate ongoing assessment and monitoring of clients to support symptom management and optimal functioning.
• Provide crisis intervention services for clients in acute distress.
• Educate and connect clients to community resources to meet needs.
• Monitor service delivery and advocate for coordinated care across providers.
• Deliver education on mental health, symptoms, and medication management to clients and families.
• Promote recovery and self-sufficiency, supporting natural supports.
