North Central Mental Health Services, Inc. is seeking a Case Manager to provide community-based behavioral health support in Franklin County, Ohio. You will coordinate care, advocate for clients, and collaborate with a multidisciplinary team to promote independent living.

The role offers full-time hours, a predictable M-F schedule, and mileage reimbursement. A 90-day paid training program and access to professional development are provided to help you grow in this essential, client-focused role.

Qualifications

• Bachelor’s Degree required (Social Work, Psychology, Behavioral Health, or related field preferred).
• 1,248 Patient Service Hours Annually (104/month).
• Valid Ohio Driver’s License & Reliable Transportation – clean driving record (≤4 points).
• Successful Background Check & Pre-Employment Drug Screening.
• Completion of 90-Day Paid Training Program.
• Experience in Mental Health or Case Management preferred but not required.

Responsibilities

• Care Plan Development: Work with clients, families and professionals to create personalized treatment plans.
• Service Coordination: Connect clients to therapy, housing, employment, and addiction recovery programs.
• Advocacy: Champion clients’ rights within healthcare, legal, and social service systems.
• Ongoing Monitoring: Track progress, ensure adherence, and adjust plans as needed.
• Crisis Intervention: Provide immediate assistance during emergencies to ensure safety.
• Education & Support: Empower clients and families with knowledge about conditions and resources.
• Collaboration: Liaise between professionals, social workers, and benefit providers.
• Documentation & Compliance: Maintain accurate EMR records and meet standards.
• Empowerment & Cultural Sensitivity: Deliver respectful, inclusive services.
