Job summary

Southeast Healthcare in Columbus, OH seeks mission-driven Case Managers to support adults with severe and persistent mental illness within integrated treatment teams. You will provide community-based services, develop resources for recovery, and advocate for patient rights.

Qualifications

• Minimum AA/BA required; LSW/LPC, CCDCII or II strongly preferred.
• Experience with SPMI and/or SUD population is preferred.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of an interdisciplinary team.
• Crisis intervention and de-escalation skills.
• Knowledge of community resources and healthcare services.
• Valid driver's license and insurance.

Responsibilities

• Provide community-based services to support patients in managing severe and persistent mental illness.
• Develop and connect patients to resources that aid in recovery, such as housing, healthcare, and employment.
• Advocate for patients’ rights and needs.
• Promote and encourage healthy lifestyle choices, focusing on both physical and mental health.
• Offer crisis intervention in emergencies to de-escalate situations and ensure patient safety.
• Work collaboratively with the interdisciplinary team to create and implement individualized treatment plans to meet goals.
• Monitor patient progress and adjust treatment plans as necessary to ensure successful goal achievement.
• Maintain monthly patient contact hours and up-to-date records of patient progress, treatment plans, and interactions.
