Job summary

Children's Advantage is seeking a Case Manager/CPST to provide Community Psychiatric Supportive Treatment services to adults, children and families in community settings across the service area. The role emphasizes collaboration with families, schools and other providers to support independence and stability.

Responsibilities include ongoing assessment, ISP coordination, advocating for clients, and delivering services outside office settings with required documentation and adherence to Ohio

Qualifications

• Bachelor’s degree in counseling, social work, psychology, or related field.
• Ability to establish and maintain effective relationships with clients and families.
• Willingness to work in community settings and travel daily within the service area.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide CPST services to adults, children, and families in community settings.
• Meet clients in homes, schools, workplaces, and other community locations.
• Assess client needs and support independence in managing basic needs.
• Coordinate the ISP and connect clients with needed services.
• Complete clinical documentation accurately and timely.
• Adhere to applicable laws, regulations, and agency policies.
