Qualifications

• Bachelor’s or Master’s degree in a Human Services field or High School diploma with 3+ years in Behavioral Health.
• Mental Health or a Human Services field and 2+ years as a Mental Health Care Manager.
• Experience in social services and behavioral health.

Responsibilities

• Provides CPST and TBS case management to adults with severe mental illness in outpatient or community settings.
• Initiates, coordinates, and monitors evaluations to identify client needs across housing, finances, benefits, health, daily living, work/education, and supports.
• Engages clients through outreach to determine needs and assists in meeting them; reevaluates outcomes regularly.

Education

Bachelor’s or Master’s degree in a Human Services field

High School diploma plus 3 years Behavioral Health experience

Mental Health or Human Services related field

Two years experience as Mental Health Care Manager
