Job summary

Primary Health Solutions is seeking a part-time Case Manager in Moraine, OH. You will deliver CPST services to adults 18+, develop collaborative treatment plans, and coordinate with a multidisciplinary team to ensure client-centered care.

Qualifications

  • Ability to read safety rules, maintain procedures, and write routine reports.

Responsibilities

  • Coordinate ongoing assessment and monitoring of clients and families.
  • Provide crisis intervention services for clients in acute distress.

Traditional Adult Case Manager Summary

The Adult Case Manager is responsible for delivering Community Psychiatric Support Treatment (CPST) services to adults age 18 and older with complex and multiple service needs. This role includes developing collaborative, goal-oriented treatment plans and supporting clients in achieving identified outcomes. The position requires effective time management, the ability to work independently in community-based settings, and the capacity to collaborate within a multidisciplinary team to ensure coordinated, client-centered care.

The Adult Case Manager works under the direct reporting of the Assistant Managers of Traditional Case Management and in guidance / collaboration with other PHS Leaders.

A Day in the Life
  • Coordinate ongoing assessment and monitoring of clients and families to support symptom management and optimal functioning.
  • Provide crisis intervention services for clients in acute distress.
  • Educate and connect clients to community resources, ensuring access to services that meet their needs.
  • Monitor service delivery and advocate for clients across providers to ensure coordinated care.
  • Provide education on mental health, symptoms, and medication management to clients and families.
  • Promote recovery and self-sufficiency, supporting development of natural supports.
  • Deliver interventions that enhance coping and daily functioning across home, work, and community settings.
  • Lead or co-facilitate therapeutic groups, aligned with Individual Service Plan (ISP) goals.
  • Meet established productivity standards (6,4800 annual minutes / 5,400 monthly).
  • Complete documentation and reporting accurately and on time, maintaining at least 90% on clinical record reviews.
  • Administer required outcome and satisfaction surveys per schedule.
  • Maintain compliant clinical records, including timely notes, service plans, updates, and required documentation per policy.
  • Complete incident reports as required.
  • Demonstrate proficiency in agency systems.
  • Attend and actively participate in all required meetings and case conferences.

Education/Experience

Experience providing case management to adults with severe and persistent mental illness and dual diagnosed populations. A Bachelor’s degree in counseling, social work, human services or a related field or an Associate’s degree in human services or a related field with at least 3 years of experience working in the field.

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. The ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills

To perform this job successfully, an individual should have the ability to gain knowledge of current practice management system, electronic medical record, Microsoft Word, text paging, Internet.

Certificates, Licenses, Registrations

Clinical licensure, Ohio nursing license.
