Job summary

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications and coordinates solving medication related problems between pharmacy, nursing, and prescribers.
• Monitors medication therapy for significant interactions and intervenes as needed.
• Evaluates and documents that selected medications achieve desired outcomes and communicates issues to the prescriber.
• Acts as a consultant in pharmacokinetics and adverse reactions to physicians, nurses and other healthcare professionals.
• Processes medication orders into the Electronic Health Record to ensure accuracy of records.
