Job summary

NAFI Connecticut, Inc. is seeking a dynamic, compassionate part-time Case Aide for our Family wraparound program.

The aide will support families at convenient locations, often the client home or community setting, and assist case managers and clinicians in delivering services. Responsibilities include guiding families toward service plan goals, coordinating transportation to meetings, and maintaining documentation in Evolv and CNNX.

Qualifications

• High school diploma required; associate's degree preferred.
• Bi-lingual (English/Spanish) applicants strongly encouraged to apply.
• Must hold a valid driver's license and have own vehicle.

Responsibilities

• Provide support and guidance to program families and youth around service plan goals.
• Support case managers and clinicians to deliver services to families.
• Connect families with service providers to help achieve goals.
• Provide transportation for families to meetings and appointments.
• Facilitate group and respite activities as needed.
• Collect required documentation for youth and families.
